What services does Confluent Health provide?

Confluent Health, along with our partnered clinics, offers Physical Therapy (PT) and Occupational Therapy (OT) to help patients get moving again. Our clinics’ primary focus is providing musculoskeletal rehabilitation. This includes Chronic Pain Management, Sports Medicine, and other conditions related to mobility.

For most Confluent Health clinics, we require an appointment and a referral. Requirements to be seen vary based on your specific clinic, so be sure to check their guidelines beforehand.

Physical Therapy can benefit patients with various health conditions. Our clinics often serve patients for injury prevention, rehabilitation, chronic pain management, and Sports Medicine.

The first appointment is typically 1 hour long to assess the patient’s needs and plan a personalized course of treatment. To ensure that patients receive the best care, physicians will review behavioral and environmental factors. Other items to consider for your first appointment include:

  • Most clinics require New Patient Paperwork, which covers personal information, insurance, and medical history.
  • Bring any necessary physical aids (ex. braces, crutches, splints)
  • Consider the symptoms or conditions that brought you to a physical therapist.
  • Consider sleep habits, activity level, and work environment.

Physical & Occupational Therapy

What conditions do you treat with physical therapy?
Physical Therapy focuses on improving the patient’s mobility, motor skills, and pain management. Therefore, our clinics are capable of treating Arthritis, Muscle Imbalance and Tension, Fractures and Sprains, Vertigo/Dizziness, Concussions, Back and Neck Pain, Pre- and Post-Natal Care, Pelvic Floor Dysfunction, and more.
Occupational Therapy focuses on recovering or improving the patient’s ability to perform everyday activities related to the central nervous system. This includes Brain Injuries, Spinal Cord Injuries, Multiple Sclerosis (MS), Parkinson’s Disease, Huntington’s Disease, Hand Injuries, and more.
Therapy can be preventative or reactive. If you have a limited range of motion (ROM), experience pain while performing ordinary tasks, or have difficulty performing tasks that are not typically hard to complete, physical therapy may be a good option for you.

Patients are typically advised to complete 2-3 visits per week over 4-6 weeks. These sessions typically last 40-60 minutes. The first visit is approximately 20 minutes longer to complete the preliminary screening and assess treatment needs.

Cognitive Rehabilitation Therapy (CRT) focuses on treating people who have sustained a brain injury or an illness that affects the brain. This type of therapy can improve a person’s ability to think through process training, strategy development, and functional applications.

Insurance and Payment

Do you accept my insurance?
Each Confluent Health partner has a list of accepted insurances available. To discover if your insurance covers our services, contact the scheduling department of your nearest clinic or speak with your insurance company about in-network and out-of-network providers.
Contact your insurance company to determine what providers and services they cover. Many insurers now offer an online search function on their website where you can locate in-network and out-of-network providers.
In addition to traditional cash and card payments, we also accept High-Deductible Health Plans, Flexible Spending Accounts, and Health Savings Accounts (HSA).
Aside from the payment options listed above, most Confluent Health partners offer a reduced, self-pay rate for those without insurance.
If you are insured, any relevant copays or deductibles will be applied once your appointment is completed. If you are uninsured, any self-pay rates will apply once your first appointment is complete.

Appointments and Scheduling

How do I schedule an appointment?
Once you receive a referral from your doctor, locate your nearest Confluent Health office or partner clinic. Once you’ve chosen your desired location, give your clinic a call to set up an appointment. The office team members will usually contact you within 24 hours to schedule your initial appointment time.
Patients can request specific providers or have one recommended for them. Patient requests are highly considered, but some physicians may be unavailable due to their current volume of patients.
Telehealth consultation availability varies by partner and clinic. However, most locations offer Home Exercise Plans (HEP) for patients to practice outside of their in-office appointments.

Patient Experience

Can I bring a family member or friend to my appointment?
Most of our clinics and partner locations allow one guest to attend therapy sessions with you. However, policies do vary by clinic, so be sure to confirm this detail when you schedule your appointment.
Confluent Health takes patient privacy seriously and abides by the Health Insurance Portability and Accountability Act (HIPAA). This act prevents healthcare providers from disclosing private, health information to unauthorized parties. For minor patients, they may be required to bring a guardian or representative to oversee care. For these patients, care provider-patient confidentiality is still established.
We do! Many Confluent Health Partners specialize in Sports Medicine and Sports PT.

Records from previous therapy sessions can be accessed through the Patient Portal. Our patient app, PTNow, tracks PT progress, sets reminders, connects patients to providers, and offers a set list of home exercises. This allows patients to maximize their treatment outside of scheduled appointments.
